Warfarin (Coumadin) tips from pharmacists

  • Take warfarin exactly as prescribed by your healthcare provider. Pay attention to the strengths, as you may have to take different strengths of warfarin tablets. You may also have to cut pills in half or take different strengths on different days of the week. 

  • As the warfarin dosing regimen can be somewhat complex, you should use a pillbox or reminder tools on your phone to ensure you are taking the correct dose. 

  • Keep a complete list of all medications you are taking to all appointments so that all healthcare providers are aware that you are taking warfarin. Wearing a medical bracelet saying that you are taking warfarin is also very helpful in emergencies. 

  • Let your healthcare provider know about any upcoming dental or surgical procedures because you will probably need to stop taking warfarin for a few days before the procedure. 

  • Green leafy vegetables such as kale and spinach, alcoholic beverages, cranberry juices, and animal liver products can alter the effectiveness of warfarin. Ask your healthcare provider for more details on a proper diet while taking warfarin. 

  • Warfarin works by preventing blood clots; thus, warfarin decreases your body’s ability to heal when you get cut or bruised. Tell your doctor immediately if you notice any unusual bleeding or bruising. 

  • If you fall and hit your head, you should get emergency medical care even if you feel well because warfarin can increase the risk of a brain bleed.
