How To Use Xarelto (rivaroxaban)

Xarelto comes in the form of a tablet or liquid suspension to be taken by mouth. If you find it difficult to swallow tablets, you can crush the Xarelto tablet and mix it with applesauce for easier oral feeding. Swallow the applesauce and Xarelto mixture immediately after preparing it - do not store it for later use. 

Whether you take Xarelto with or without food depends on the condition being treated.

  • If you have nonvalvular atrial fibrillation and take Xarelto to prevent stroke, you should take it once a day with your evening meal. 
  • If you take it to treat DVT or PE, you should take Xarelto with food.
  • If you take Xarelto to prevent DVT or PE, you can take it with or without food. 
  • If you are taking Xarelto after hip or knee replacement surgery, it is usually taken once a day with or without food. 
  • If you take Xarelto after hospitalization to prevent blood clots, you can take it once daily with or without food.
  • If you have coronary artery disease or peripheral artery disease and are taking Xarelto, you should take it twice a day with or without food.

Follow the directions on your prescription label for the correct Xarelto dosage and frequency. Take your medication at around the same time every day. Do not take more or less than the prescribed dose or take the medicine more or less frequently than prescribed.

Continue taking Xarelto even if you feel well. Do not stop taking this medication without talking to your doctor. If you discontinue Xarelto too early, you will be at increased risk of blood clots and stroke. Be sure to refill your Xarelto prescription before running out, so you don’t miss any doses. If you want to come off Xarelto, talk to your doctor about taking an alternative anticoagulant (blood thinner medicine) to reduce your risk of stroke and blood clots.

Missed dose: If you miss a dose of Xarelto, take the medicine as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and take your next dose at the regular time. Do not take an extra dose to make up for a missed dose. Taking two doses of Xarelto together can increase the risk of bleeding. However, if you are taking Xarelto to treat DVT or PE and you forget a dose, you can take two doses at the same time and then continue taking Xarelto according to your regular dosing schedule the next day.

Storage: Store Xarelto at room temperature (between 15-30° C or 59-86° F) away from moisture, heat, and direct light in a tightly closed container. Keep all medications out of reach of children and pets.

Unused medicine: Throw away any expired or unused Xarelto. Do not flush the medicine down the toilet. The best way to dispose of medicines is through a medication take-back program. Learn more about proper medication disposal
